Wyoming EPSCoR Undergraduate Fellowship Survey 2008

Wyoming EPSCoR is conducting a new evaluation of its NSF EPSCoR Undergraduate Fellowship Program.  Your response is very important to the future of the program. We are very interested in hearing from all former students who were Wyoming NSF EPSCoR Fellows, whether you have continued in an area of science, math, or engineering, or whether you have chosen another career path.  We would like to know how your experience with undergraduate research influenced you and any ideas you may have for improving the program.

We would like to have your response October 3, 2008 if possible.  We are in the process of preparing a new proposal to the National Science Foundation, and your survey will provide essential information for the continuation of the program and its funding.

You will need Adobe Acrobat Reader, version 7 or later to use the form. You can visit http://www.adobe.com/  to download a free copy of the software.      You also will need email software loaded on your computer.

This form can be typed in, saved, and submitted directly by e-mail:

  • Please save the form to your hard drive and complete it.  You should be able to save the completed form.  Then attach it to an email and send it to Rick Matlock, rixdogs@uwyo.edu/. If you choose, you may send a copy of your Curriculum Vitae or résumé along with your survey. If you wish to send a copy of your CV or résumé, attach it to the email.  Two other buttons will allow you to save your completed form to your hard drive and also print a copy for your records.  Some email systems will allow you to click on the “Submit by Email” button, and an email message addressed to a member of the EPSCoR staff should open automatically with your form attached.  If you have problems with the form, please contact Rick Matlock, rixdogs@uwyo.edu.
